What is WALC?
The goal of the WALC Program is to engage, empower and activate Medicaid and Medicaid-eligible residents of the South End, West Hill and Arbor Hill (and nearby neighborhoods) to make consistent use of the preventative health care and wellness systems and to advocate with the health care system to promote integrated, culturally and linguistically competent accessible primary medical care access.

WALC hires community health workers who support their neighbors in achieving health and wellness goals. WALC staff provide social determinant of health resource navigation, case management, and other wrap around services. Services are tailored based on acuity, need, and community member goals.
Expected Outcomes: Each community member who engages with WALC can except to achieve these three outcomes:
- Secure active health insurance
- Engagement with primary care (at least one annual wellness visit with a primary care doctor)
- All pharmacy supports met
